8. Between 2022 and the present date: a. On how many occasions did the NHMRC investigate a potential breach of any of the required codes or guidelines? b. How were each of these potential breaches brought to the NHMRC's attention, and what was the potential breach investigated in each case? c. On how many occasions did the NHMRC find that a breach of one of these codes or guidelines had occurred? d. On how many occasions did the NHMRC take any action under the Funding Agreement in response to a potential or identified breach of one of these codes or guidelines? e. What action was taken in each of these cases?
