With reference to the Inspector of Detention Services’ ‘Combined inspection report for youth detention centres’ dated November 2025 – Will the Minister advise (a) if any disciplinary action has been taken in relation to the inaccurate recording of observations and welfare checks undertaken at West Moreton Youth Detention Centre, referred to on page 84 of the inspector’s report and (b) what steps have been taken to address inaccurate recordkeeping during separations?
